What happens to your ongoing care?


You have the option to either continue your care with Dr Dahiya at his new practice.

If you wish to continue seeing Dr Dahiya and you're from a clinic outside of Cygnet, you will need to contact Frontier Advanced Medicine directly using the contact details above.

Dr Dahiya’s new practice will be responsible for the ongoing management of your care, including appointments, prescriptions, referrals, correspondence and medical records.




Outstanding enquiries and requests


Cygnet Clinic has done its best to finalise outstanding enquiries and requests before Dr Dahiya’s departure. A list of pending medical records requests and all other outstanding tasks has been provided to Dr Dahiya.





Medical records


Dr Dahiya has ongoing medico-legal and professional obligations in relation to the medical records of his patients. Accordingly, responsibility for the management and provision of access to Dr Dahiya’s patient records will transfer with his independent practice.




Existing appointments


If you have an existing appointment with Dr Dahiya from a clinical outside of Cygnet, on or after 1 October 2026, please contact Frontier Advanced Medicine directly to confirm or manage your appointment.  If you have been seeing Dr Dahiya before 1 October 2026 he will remain bookable until the 31 October 2026.


Any changes, cancellations or rescheduling of appointments with Dr Dahiya will need to be arranged through his new practice.
