Shortcut to Booking Forms

The Reception Psychology Intake forms here are for referrals where the clinician is already assigned (the standard intake form) and this process guides reception to collecting relevant information to set up their file in Halaxy and Folder in Google Drive. 

The unallocated referrals that would normally be put on a waitlist should be processes with the Unallocated Referral Board Intake Form instead

Psychiatry Intake Forms

Unlike Psychology, there is no direct standard bookings unless directed otherwise.  Each referral needs to be placed on the referral board in order to be reviewed before accepted
